Freeport School District Registration (2012 - 2013)
Registration for the 2012 - 2013 school year will begin April 9th and continue through April 27th.
If you are new to the district and are unsure which school your child will attend, please contact the Freeport School District central office at (815) 232-0300.
New students and existing students require different forms for registration. A new student would not have attended FSD145 schools during the prior school year.
Please understand the following instructions before sending your completed registration forms to your child's school:
1) One set of forms must be completed for each child being registered
2) All forms must be printed, signed, and returned to the school your child will attend.
3) Parents may choose to enroll their child in a half-day Kindergarten option. Parents choosing to enroll their child in half-day Kindergarten must still complete all registration forms. There is no "short" form for half-day Kindergarten.

Course and Book Rental Fees

Book rental fees may be paid at any time.
Middle School and High School course and fee schedules will be distributed prior to your child attending school. Please do not pay for elective courses until you have received your child's schedule.

Important: Please retain a copy of all forms for your records before mailing or dropping the originals off at your child's school.
If you have questions about how to complete the registration forms or which forms you are required to complete for your child, please contact the building secretary of the school your child will attend.
Completing the registration forms requires Adobe Reader. If you receive an error opening the files, check to make sure that you have the correct software installed.
